Documents to Bring to Your Appointment

WRAP – WATER ASSISTANCE

  • Valid Michigan State issued Picture ID for everyone 18 and older in household with current address
  • Social Security Cards for everyone in household
  • All household income received for everyone 18 yrs. and older for the last 30 days from date of appointment, including check stubs, proof of paid child support received, DHHS cash assistance award letter, pension, SS/SSD/SSI and SSI Subsidy (received quarterly) award letters
  • Current water bill with account holder’s name (Detroit) and address matching picture ID, also Detroit residents MUST have the account holder’s name on the bill as of July 1, 2018
  • Home Owners: proof of home ownership including current tax bill, deed or current mortgage statement. Land contract
  • Renters: lease matches ID, address, signed by Landlord and tenant, states tenant is responsible for water bill (Only necessary for clients whose bill is listed to resident, occupant, or customer)

DTE – UTILITY ASSISTANCE (MEAP)

  • Valid Michigan State issued Picture ID for everyone 18 and older in household with current address matching service location
  • Current Gas/Electric bill with name and address matching picture ID
  • SER Determination Notice

TAX APPOINTMENTS

  • Your Social Security Card and cards for all claimed dependents (Originals Only)
  • Valid Picture ID (Driver’s License or State ID)
  • Spouse (If filing Joint Return)
  • Bank or Credit Union routing and account numbers (For Direct Deposit)
  • Copy of last year’s tax returns
  • Completed Intake Form
  • Proof of income
  • Wages (all forms W‐2)
    • Unemployment (Form 1099‐G)
    • Gambling/Lottery Winnings (Form W‐2G)
    • Miscellaneous Income (Form 1099‐ Misc)
    • Pension (Form 1099‐R)
    • Social Security Benefit (Form 1099‐SSA)
    • Supplemental Security Income or Letter from SSA
    • VA total monthly benefit or sum for the year
    • Interest and Dividends (Form 1099‐INT and/or 1099‐DIV)

  • Medical or HMO Premiums Paid for self or dependents
    • Paid daycare costs and provider’s Tax I.D. number
    • Student Loan Interest (1099E)
    • Tuition (1098T)
    • Health Care Coverage (Form 1095‐A, B, or C)

Special Elections

    • Home Heating Credit
        – Total amount of residential heating costs paid for the tax year

    • For Homestead Property Tax Credit: Homeowners
        – Proof of taxable value of your home and bills for summer;/winter property taxes paid
        – Mortgage Interest (Form 1098)

    • Renters
        – Lease or rental contract and rent receipts
        – Landlord’s name, address, and zip code

WEATHERIZATION

The following documents will be needed for Weatherization Intake:

  • Identification with current address
  • Social Security Cards and Birthdate date for everyone in the household
  • All proof of gross income for the past 90 days/3 months for all household members with income (no bank statements or W2s)
    – Wages
    – Unemployment (Form 1099‐G)
    – Gambling/Lottery Winnings (Form W‐2G)
    – Miscellaneous Income (Form 1099‐ Misc)
    – Pension (Form 1099‐R)
    – Social Security Benefit (Form 1099‐SSA)
    – Supplemental Security Income or Letter from SSA
    – Quarterly SDA statement
    – VA total monthly benefit or sum for the year
    – DHS Assistance (DHS Annual Statement)
    – Other relevant income information
    – Child support
    – FAP Award Letter/Bridge Card Award Letter
  • Proof of home ownership and Co-Owners info
    – Winter or Summer Property Tax Bill
    – Quick claim Deed
    – Mortgage statement
    – Property Assessment

    (If making payments on past due taxes a Signed “Stipulated Tax Foreclosure Avoidance Agreement, IRSPA Regarding Delinquent Taxes”, also proof of those payments.)

    • Most recent Gas and Electric Bill.
    • Renters must provide landlord/property management information: Name, address and phone number at appointment

    An application cannot be started and completed without all required documents.

FINANCIAL COUNSELING

What to have on hand (not mandatory, but helpful):

  • Social Security Number (For pulling credit)
  • Bank Statements
  • Pay Stubs/All Sources of Income
  • Household Bills
  • Government issued ID

PROPERTY TAX COUNSELING

What to have on hand (not mandatory, but helpful):

  • Most recent property tax bill
  • Completed Poverty Tax Exemption(PTE) Application from your local assessor’s office 

PROPERTY TAX EXEMPTION

To apply, you will need:

Proof of Ownership

  • Deed, Land Contract or Court Order for your home
  • Current photo ID for all people listed on Proof of Ownership
  • Death certificate for any owner who is deceased

Proof of Residence

  • For all adults (18+ years) who live in the home: Current photo ID or driver’s license
  • For all children (5 – 17 years) who live in the home: Current report card, transcript, healthcare record or government document with name and address

Tax Documents

For all adults in the home who file income tax returns:
– Copy of Federal income tax return for the last year
– Copy of State income tax return for the last year

For all adults in the home who do not file income tax returns:
– IRS form 4506-T (comes with your application)
AND
– Poverty Exemption Affidavit (4988) (comes with your application)

For the household, if applicable:
– Property Tax Rebate
– Home Heating Credit
– Other Tax Credits

Proof of Occupancy

  • Water Bill
  • DTE bill (front & back)

Proof of Income

For everyone who lives in the home:

  • W-2 or paystub
  • Unemployment Benefit (1099-G)
  • Pension (W-2 or 1099R)
  • Social Security or Supplementary Income (SSI/SSA-1099) Benefit Letter

  • FIA or DHS Award letter
